The women's 4 × 400 metres relay at the 2021 European Athletics Indoor Championships was held in a single round at the Arena Toruń in Toruń, Poland, on 7 March 2021.

The race was won by the team of the Netherlands in a championship record and national record of 3:27.15 minutes, followed by the teams of Great Britain and Northern Ireland in 3:28.20 minutes and Poland in 3:29.94 minutes.

Outside the medals, the team of Italy set a national record in 3:30.32 minutes in fourth place and the team of Ukraine set a national record in 3:30.38 minutes in fifth place.
