O'Sullivan was a dual player at club level, lining out with Blackrock in hurling and St. Vincent's in Gaelic football.
O'Sullivan's club hurling career coincided with a 25-year drought for Blackrock.

O'Sullivan died at Heatherside Hospital in Buttevant, County Cork on 7 January 1996.
