This is a list of award winners and league leaders for the Philadelphia Phillies professional baseball team.

The players were honored on August 5, 1969, at Connie Mack Stadium before the Phillies' game against the San Francisco Giants.

The players were honored on September 27, 2003, prior to the penultimate game at the stadium, which the Phillies went on to win against the Atlanta Braves 7–6.

All the other honorees attended, including Tug McGraw, who was recovering from brain surgery.
