[1] Forman began his career at Nottingham Forest where he played in five matches between 1900 and 1902.
[1] After a spell with non-League club Sutton United the outside left signed for Barnsley.
[3] In 1910 he joined Tottenham Hotspur, featuring in a further eight matches and scoring a single goal before ending his career at Sutton Town.
[1][4][5] His solitary goal on his Lilywhites debut in a 6-2 victory over Middlesbrough at White Hart Lane in February 1911 in the old First Division.
Forman served as a lance bombardier in the Royal Garrison Artillery during the First World War.
