As the vehicles are about to collide with a tractor trailer, it is revealed that all the events of the movie were actually a drug-induced hallucination, imagined by an overly medicated, soon to be released mental patient named Donald, who fantasized himself as the clown and his nurse Fay as his alter-ego's love interest.
