Also injured was Ernest Day, aged 24, of Cardiff, fireman on the mineral engine.

According to the Board of Trade report,[2] the accident was caused by the signalman in Park Junction signal box pulling the wrong lever.

This allowed the heavy goods train onto the down gradient and cross the main line into the path of the passenger train at Ebbw Junction.

Even though he realised his mistake and returned the signals to danger, there was not enough time for either train to avoid the collision.
