Zirka-2 Kirovohrad won on walkover after Kharchovyk Popivka withdrew from the play-offs.
Tysmenytsia won 4–1 on aggregate.
Shakhtar Horlivka won on walkover after Hirnyk Pavlohrad did not arrive.
